Waiver form for try-outs 2011-2012
Potential Laker Dance Team Members: If you are planning on attending try-outs for the 2011-2012 season on April 30th and May 1st, please make sure you print off the waiver form, fill it out, and bring it with you to try-outs. Once again, the items you NEED to have with you on that day include:
- Completed waiver form
- For current GVSU students: class schedule for the 2011-2012 academic year and evidence showing your gpa is an average of 2.0 or higher
- Incoming freshman or transfer students: proof your your acceptance to GVSU for the 2011-2012 academic year
- $25 non-refundable registration fee
